Employer Response to Employee Request for FMLA  (Form WH-381)

Initial form to be given to an employee who has requested FMLA leave; who is in a situation that you think might qualify as FMLA leave; notifying them of their eligibility or ineligibility.

FMLA- Certification of Health Provider- Employee's Serious Health Condition  (Form WH-380E)

To be completed by employee's medical provider for employee's own serious health condition.

FMLA- Certification of Health Care Provider  (Form WH-380F)  Family Member Serious Health Condition

To be completed by medical provider to document a family member's serious health condition.

FMLA- Designation Notice to Employee of FMLA Leave  (Form WH-382)

To be completed and given to employee once supporting documentation is received and reviewed by HR and the qualifying reason for leave is validated.
